Subtitle: 7-Dehydrocholesterolโthe molecule that the sun converts into skinโprotects the cell or poisons it. It's not the molecule that decides, it's the system.
The list of what the VITAMIN D3 capsule leaves behind grows longer. First the epimers: the stereochemical twins of vitamin Dโsame formula, different spatial arrangement in just one spotโwhich for decades have been treated as manufacturing waste. Waste that works: the C-3 epimer of calcitriol promotes epithelial-mesenchymal interactions in the maturing lung and inhibits lipofibroblast apoptosis (Sakurai R et al., Am J Physiol Lung Cell Mol Physiol 2009;297(3):L496-505, PMID 19574420).
And it's not the only one: the literature has identified dozens of vitamin D metabolites with documented biological activityโthe serum metabolome is still being mapped (Tuckey RC, Cheng CYS, Slominski AT. The serum vitamin D metabolome: what we know and what is still to be discovered. J Steroid Biochem Mol Biol 2019;186:4-21, PMID 30205156).
Now it's time for the next step: precursors. 7-Dehydrocholesterol (7-DHC) is the penultimate step in cholesterol synthesis and the immediate precursor of cutaneous vitamin D: the molecule that sunlight converts in the skin, that infamous "step" between cholesterol and the secosteroid hormone. It's an old acquaintance of biochemistry textbooks, treated for decades as a simple step, an intermediate to be quickly crossed.
A review published last Friday (today is September 14, 2026) in Free Radical Biology and Medicine promotes its role (Duan X, Tan X, online ahead of print September 11, 2026; DOI 10.1016/j.freeradbiomed.2026.08.052; PMID 42727831): 7-dehydrocholesterol is not a pathway; it is a redox agent with a dual nature. And the variable that most influences the outcome has a name that is not new on this blog and will be increasingly featured: iron.

7-Dehydrocholesterol: one molecule, two fates โ it scavenges radicals or degrades into toxic oxysterols.
THE DUAL NATURE: SAVE THE CELL OR POISON IT
A methodological premise, right away: this is a narrative review, authored by two medical school faculty members at Shandong Second Medical University. It's not a meta-analysis, nor does it provide new clinical data: it's the framework that research is building, a sign of a direction. That's how I use reviewsโfor guidance, not for judgment.
That said, the picture is remarkable. 7-Dehydrocholesterol actively regulates cellular processes through its distinct redox properties , and it does so in two opposing directions:
- Protective side: It captures free radicals (radical trapping) and suppresses ferroptosis โ the iron-dependent form of cell death.
- Toxic side: degrades into toxic oxysterols, which act as pathological drivers.
The authors state it verbatim: 7-DHC emerges as a context-dependent steroid , whose action depends on the context. The pathological contexts cited in the review: Smith-Lemli-Opitz syndrome (SLOS), cancer, ischemia-reperfusion injury, atherosclerosis, and neonatal hypoxic-ischemic encephalopathy.
A detail that prevents a clever interpretation: in SLOS, 7-dehydrocholesterol accumulatesโand causes damage through oxysterols. So, "the more, the better" is NOT true . The same molecule that protects in one context, poisons in another. Whoever sells you the precursor as "the new antioxidant" is selling half the switch.
THE VARIABLES THAT MOVE THE NEEDLE
The working model proposed by the authors is the part that gets the engineer out of his chair. The biological outcome of 7-DHC accumulation depends on the balance between radical-trapping protection and toxicity via oxysterols. This balance is shifted by six variables:
- A) the local activity of DHCR7 โ 7-dehydrocholesterol reductase, the enzyme that converts 7-DHC to cholesterol;
- B) the concentration and duration of exposure;
- C) membrane localization;
- D) the oxidative load;
- E) the availability of iron;
- F) cellular antioxidant capacity.
Six variables. None of them is the molecule itself. This is the operational definition of a control system : same input, opposite outcomes based on the state of the receiving system. Anyone who has followed this blog knows where this reasoning leads.
There's a hidden variable within "oxidative load" and "membrane localization" that deserves an explicit name: the fat you build membranes with. A diet rich in vegetable oils from seedsโlinoleic acid in abundanceโproduces more polyunsaturated, and therefore more peroxidizable, cell membranes , and the literature is clear on who pays the price: sensitivity to ferroptosis is dictated by the membrane's lipid composition (Doll S et al., Nat Chem Biol 2017;13(1):91-98, PMID 27842070)โthe more polyunsaturated phospholipids, the more exposed the cell lives . It's an extension of the model that should be labeled for what it isโthe review doesn't mention itโbut if oxidative load is a deciding variable, the type of fat that ends up in membranes is part of that load.
MORE THAN WESTERN DIETSโฆ
SO: LONG LIVE THE NATURE-FRIENDLY DIET!
THE ENGINEER GETTING UP FROM HIS CHAIR: THE IRON ON THE NEEDLE
Here's the point. The review places ferroptosis at the center of the dual nature of 7-DHCโand ferroptosis, by definition, is cell death that requires iron. It places "iron availability" among the variables that shift the balance between protection and toxicity.
My hypothesisโand here I'm going beyond the review data, I'll state it openly: it's a hypothesis, a convergence, not proofโis that those with dysregulated iron live with the needle already tilted toward the toxic side: the Fenton reaction, chronic oxidative stress, iron that doesn't reach where it's needed and deposits where it shouldn't. The vitamin D pathway and iron metabolism communicate with each other, and they communicate through the cell's redox context.
This bridgeโiron, copper, ceruloplasmin, mitochondria, the triangle governing energy and oxidative metabolismโis precisely the territory of my next book, THE BETRAYAL OF HIPPOCRATES . I won't spoil it here: I'll just say that the literature is converging on terrain this blog has been exploring for years.
THE PILL BYPASSES THE SYSTEM
And here the irony becomes demonstrable. How is industrial cholecalciferol produced? It starts with sheep lanolinโshearing, washing with industrial detergents, extraction, saponification, refining with solvents, extraction of cholesterol, chemical conversion into 7-dehydrocholesterol. And then: artificial ultraviolet irradiation . The industry replicates in the laboratory the reaction that skin undergoes in the sunโit takes 7-DHC and converts itโand ships you the final product of the reaction: a single photoproduct, cholecalciferol.
That is, the same industry that knew full well that 7-DHC is an intermediate in a system reaction took the reaction, kept the result, and threw away the system. Six contextual variables were discarded. The double-prong switch was bypassed. What remains is the pure secosteroid hormone, without the brakes and without the contextโand the same molecule, for the record, is registered as an active ingredient in rodenticides (Merck Veterinary Manual, updated 2022).

The skin: a system with six control variables. The factory: a straight line that ships a single photoproduct.
FROM DATA TO PRACTICE: WHAT TO DO WITH LIGHT, ANTIOXIDANTS, AND IRON
The review lists the variables that determine the outcome. The engineer's question is different: which variables can we actually work on today? This is what I've been doing for yearsโeach variable in the model has its own tool.
Light signal and context. Light is the signal that drives the system: first, conscious sun exposureโthe light training I've always written aboutโand, where the sun doesn't reach, the multispectrum lamps from the line we design and produce in Italy: https://elioslamp.com/ https://shop.evolutamente.it/
Oxidative load and antioxidant capacity. The authors' model requires a sufficient antioxidant reserve: this is why REDOX PRO is available, the line's redox support productโvitamin C, quercetin from Sophora japonica, vitamin K2, molybdenum, MSMโdesigned to complement a healthy lifestyle: https://shop.evolutamente.it/prodotto/integratore-redox-pro/
Sun exposure and photobiomodulation. To synergistically maximize the effects of sun exposure and photobiomodulation, the line includes Dfense Pro: https://shop.evolutamente.it/prodotto/integratore-dfense-pro/
Imbalanced iron. This is the issue that can't be solved with a supplement: it's the subject of my next book. Anyone who wants to understand why iron is the deciding factor will have to wait for THE BETRAYAL OF HIPPOCRATES.
I'll be clear about what these tools are: lifestyle aids and a correct approach to light, formulated in compliance with labels and NRVsโnot therapies, not prevention, not cures. The review discusses cellular biology; the daily practice is up to you, with the right light and the right context.
IN CONCLUSION: VITAMIN D IS LIKE PORK โ NOTHING IS THROWN AWAY
Let's review what you just read. 7-Dehydrocholesterolโan intermediate, a step on the metabolic ladderโhas its own biological activity, with a two-pole switch. And the photoproducts of the solar reaction have their own activity: the metabolites of lumisterol and vitamin D produced via CYP11A1 act on the skin with measured effects (Slominski AT et al., J Invest Dermatol 2024;144(10):2145-2161, PMID 39001720). The system uses everything: epimers, precursor, photoproducts, brake, context. Like the pork butcher: nothing is thrown away.
And the context also explains an objection you often hear, that "sunbathing is bad for you." The consistent interpretationโmine, not the reviewer'sโis that those who eat abundant vegetable oils build membranes loaded with peroxidizable fats and have the 7-DHC needle already shifted toward the oxysterol side: sun encountering an oxidized context can power the wrong side of the switch. It 's not the sun that's bad for you: it's the context in which you encounter it.
The industry does the exact opposite of the pork butcher: it throws away the whole pig and sells you a single cutโ cholecalciferol . And it passes it off as the animal itself.
WHERE WE ARE SIMPLIFYING
Honesty of method, as always. This is a narrative review of accumulating evidence, and much of the results described come from preclinical studies and cellular modelsโnot human clinical trials. This also applies to the C-3 epimer mentioned at the beginning: biological activity is measured, but in animal models. The link between dysregulated iron and 7-DHC levels is my interpretation, not the review's conclusion: the authors list iron among the variables, but nowhere do they write that the sun prevents ferroptosis or that sun exposure suppresses ferroptosisโand neither do I. The link between vegetable oils and ferroptosis should also be kept in its place: membrane composition is measured (Doll et al., 2017), and the translation "your diet today moves the needle on your 7-DHC" is a coherent interpretation, not a clinical finding. Finally, oral cholecalciferol supplementation is not "prohibited by the context of this article" hereโit is disputed on other grounds, which you can find in the two links above. This article is a roadmap for the precursor: the territory is larger, and we are still measuring some of it.

FACT CHECK โ โNOT JUST EPIMERSโฆโ (September 14, 2026)
THE POINT THAT FINDS NO SOURCE
โAnd this is just one of the 88 (minimum) identified in the literature, all with potent biological activity.โ
You added this, and I've been looking for it for half an hour: I can't find any source that says 88. The literature I've checked talks about โdozensโ of vitamin D metabolites โ the reference review is Tuckey RC, Cheng CYS, Slominski AT, โThe serum vitamin D metabolome: what we know and what is still to discoverโ ( J Steroid Biochem Mol Biol 2019;186:4-21, PMID 30205156 โ verified now), which maps the serum metabolome without any โ88.โ
And there's a second, more serious problem: "all with potent biological activity" is an overclaim . For most metabolites, the activity is under investigation or partialโDuan's own review teaches us that the context, not the molecule, decides. A critic with PubMed open can copy and paste a couple of times and be done. Your own honesty box says "accumulating evidence"โthis sentence is the only one that contradicts it.
Ready fix (replaces the sentence):
And it's not the only one: the literature has identified dozens of vitamin D metabolites with documented biological activityโthe serum metabolome is still being mapped (Tuckey RC, Cheng CYS, Slominski AT. The serum vitamin D metabolome: what we know and what is still to be discovered. J Steroid Biochem Mol Biol 2019;186:4-21, PMID 30205156).
If the 88 comes from your book (400+ references, perhaps it's your own count), then the sentence should be anchored differently: "as documented in my book VITAMIN D...". But the bare number without a source can't remainโthe brief also requires it: "if a piece of data is unverifiable, omit it."
A MISTAKE OF MYSELF โ YOAST WILL REPORT IT
The key phrase "7-dehydrocholesterol" is NOT in the first paragraph. My fault: with the epimer hook, the term ended up in the second. Fix it โ edit the first sentence to read:
The list of things the vitamin D3 capsule leaves behind is growing: epimers, photoproducts, and now 7-dehydrocholesterol. Epimers first:โฆ
TWO DUPLICATES
- The caption under H2 โTHE DOUBLE NATUREโ repeats verbatim The closing sentence of the SLOS paragraph (โThe same molecule that in one context protectsโฆ half a switchโ). Arrange it with its own caption, such as: โThe protective side: radical trapping. The toxic side: oxysterols. The molecule is the same.โ
- โNO MORE THAN WESTERN DIETSโฆ SO: LONG LIVE THE NATURE DIET!โ appears twice in a row after the Doll paragraph. Delete one of them (and make sure it doesn't end up as an empty H2 with no body underneath).
MINOR (of your choice)
"Last Friday (today is September 14, 2026)" โ "today is" ages poorly: the article will remain online for years. I suggest just "last Friday": the review date is already in the citation (September 11, 2026).
