The SARS-CoV-2 virus has undergone many mutations since early 2020. Based upon current understanding of new variants that involve mutations in the nucleocapsid protein that can undergo liquid-liquid phase separation, the September 2021 Guidelines for melatonin dosage requires extensive revision. It is now crystal clear that both the timing and amount of melatonin used is critical in controlling viral replication and infection upon exposure. Inadequate amount and delayed application can extend symptom durations and potentially even result in the development of many long-term symptoms associated with LongCovid otherwise known as Post-Acute Sequelae of COVID-19 (PASC).
The April 2023 Revised Guidelines are derived from the Weight-Based (24 h) Dose (WBD) schedule. The WBD dose is allometrically scaled from proven effective animal doses under different parameters and infection status by various highly pathogenic viruses that also undergo phase separation [1]. The rationale for higher doses is based upon the concept that higher concentration is required to inhibit viral phase separation that supports replication and virulence [1,2]. Thus a higher MEL dose may mean reversing and controlling symptoms within 2 to 12 hours or days and weeks at inadequate doses.
